Patient Portal

As a new or existing patient, you are able to fill out your medical history forms online through this Patient Portal.

To create an account, you must:

1. Have an existing scheduled appointment
2. Select the Patient Portal link, click “Sign Up Today”

After you create an account, you will then have the ability to:

1. Fill out and submit your medical history forms online to save time while checking in
2. Check in online 4-6 days before you’re scheduled appointment*
3. Access secure messaging to ask medical questions to your provider or refill your prescriptions
4. Sign on and view your upcoming appointments
5. Pay bills with a click instead of a check

*Please note, patients will need to see a front desk member after completing the online check in.
